Global Developmental Delay
A diagnosis used for children under 5 who fail to meet expected developmental milestones across several areas of intellectual functioning, when severity level cannot yet be reliably assessed through standardized testing.
Global Developmental Delay involves significant delay across multiple developmental domains in a child too young for reliable standardized testing; Specific Learning Disorder involves a specific, circumscribed difficulty in one academic skill domain (reading, writing, or math) despite average intelligence and adequate instruction.
Global Developmental Delay applies before age 5; Specific Learning Disorder typically becomes clinically apparent once formal academic instruction begins, usually school age.
Both require persistence beyond what's expected for age and developmental stage, not a transient lag.
Global Developmental Delay affects multiple domains broadly (motor, language, cognitive, social); Specific Learning Disorder is, by definition, circumscribed to a specific academic skill despite otherwise average intellectual functioning.
Global Developmental Delay's impairment is broad and pervasive across domains; Specific Learning Disorder's impairment is specific to the academic skill area affected, with other domains intact.
Specific Learning Disorder specifically excludes cases better explained by intellectual disability or global developmental delay -- a circumscribed academic difficulty within an otherwise broadly delayed profile points toward Global Developmental Delay instead.
The key differential question is breadth: is the delay confined to one academic skill domain with intelligence and other domains intact (Specific Learning Disorder), or is it broad across multiple developmental domains (Global Developmental Delay)?
A young child with broad developmental delays that happen to include early academic-readiness skills can be mistakenly evaluated for Specific Learning Disorder before the broader, more accurate picture of Global Developmental Delay is recognized.
A 4-year-old with delays in motor skills, language, and social play, plus slow acquisition of pre-academic skills, points to Global Developmental Delay; a 7-year-old who speaks, socializes, and moves at age level but persistently struggles specifically with reading despite adequate instruction points to Specific Learning Disorder.
| Feature | Global Developmental Delay | Specific Learning Disorder |
|---|---|---|
| Breadth of delay | Global Developmental Delay: multiple domains | Specific Learning Disorder: one academic skill domain only |
| Overall intellectual functioning | Not yet reliably assessable | Average, by definition |
| Typical age at diagnosis | Under 5 | School age, once academic instruction has been provided |
A diagnosis used for children under 5 who fail to meet expected developmental milestones across several areas of intellectual functioning, when severity level cannot yet be reliably assessed through standardized testing.
Persistent difficulties learning and using academic skills (reading, written expression, and/or mathematics), with performance well below what's expected for age, present despite adequate instruction, and not better explained by intellectual disability, uncorrected sensory problems, or other neurological/mental conditions.
