Illness Anxiety Disorder
A preoccupation with having or acquiring a serious illness, in the absence of significant somatic symptoms, driven by anxiety about health rather than by the physical symptoms themselves.
Illness Anxiety Disorder is a specific, criteria-based diagnosis: preoccupation with having or acquiring a serious illness, with somatic symptoms absent or mild, persisting for at least 6 months, with high health-related anxiety and either excessive health-related behaviors or maladaptive avoidance. Other Specified/Unspecified Somatic Symptom and Related Disorder is the chapter's residual category, used when somatic- or illness-related symptoms cause clinically significant distress or impairment but do not meet full criteria for Illness Anxiety Disorder or any other specific disorder in the chapter.
Illness Anxiety Disorder's onset is tied to the emergence of persistent, criteria-meeting health-related anxiety. The residual category has no fixed onset pattern, since it covers atypical presentations at any point in the course.
Illness Anxiety Disorder requires the preoccupation to persist for at least 6 months. The residual category is used specifically when a presentation resembles Illness Anxiety Disorder but falls short of this duration threshold, or otherwise diverges from its specific criteria -- for example, illness preoccupation present for less than 6 months.
Illness Anxiety Disorder's features are anchored to its specific criteria: high health-related anxiety, easy alarm, and either excessive care-seeking or avoidance behavior. The residual category's features vary by the specific atypical presentation involved.
Both require clinically significant distress or impairment. In Illness Anxiety Disorder, this is tied to the full-criteria illness-preoccupation pattern. In the residual category, distress or impairment is present even though the duration or other specific criteria for a named disorder are not fully met.
Illness Anxiety Disorder requires meeting its specific 6-month duration threshold and symptom profile. The residual category is used only after Illness Anxiety Disorder and the chapter's other specific disorders have been considered and full criteria are not satisfied.
When illness-related preoccupation meets the full criteria (duration, symptom profile, and behavioral pattern), Illness Anxiety Disorder applies. When illness-related anxiety is clinically significant but present for less than 6 months, or otherwise doesn't fit the full criteria, Other Specified Somatic Symptom and Related Disorder (illness anxiety disorder without excessive health-related behaviors, or of less than 6 months' duration) is the more accurate diagnosis.
A common error is diagnosing full Illness Anxiety Disorder as soon as health-related anxiety appears prominent, without checking the specific 6-month duration threshold -- a presentation that is clinically significant but below that threshold belongs in the residual category, not automatically upgraded to the full disorder.
A person has been preoccupied with the belief that she has cancer for the past 10 months, checking her body daily and avoiding medical appointments out of fear -- meets full Illness Anxiety Disorder criteria. A person develops significant health-related anxiety about a possible illness over the past 2 months, following a family member's diagnosis -- Other Specified Somatic Symptom and Related Disorder (illness anxiety disorder of less than 6 months' duration).
| Feature | Illness Anxiety Disorder | Other Specified / Unspecified Somatic Symptom and Related Disorder |
|---|---|---|
| Duration threshold | At least 6 months | Below that threshold, or otherwise atypical, but still clinically significant |
| Diagnostic role | A specific, criteria-based diagnosis | The deliberate fallback, including named 'less than 6 months' and 'without excessive health-related behaviors' examples |
| Behavioral pattern | Excessive health-related behaviors or maladaptive avoidance required | Varies by the specific atypical presentation |
A preoccupation with having or acquiring a serious illness, in the absence of significant somatic symptoms, driven by anxiety about health rather than by the physical symptoms themselves.
A residual classification for presentations with somatic symptoms or health-related distress and behavior causing clinically significant distress or impairment that do not meet full criteria for Somatic Symptom Disorder, Illness Anxiety Disorder, Functional Neurological Symptom Disorder, or Factitious Disorder.
